Support for older people with complex health needs

Navigating health and social care systems is particularly difficult for older people with complex health needs, including those with dementia, frailty and multimorbidity.

This group is particularly at risk of poor continuity and fragmentation of care and are often reliant on support from others, typically family members.

The goal of this research is to identify appropriate models of care for older people with complex needs, including those that foster integration, continuity and the involvement of patients and family carers in decisions about their care.

This includes studies on:

  • interventions to support integration and inter-professional working.
  • addressing the challenges faced by people living with dementia and other co-morbid health conditions.
  • promoting partnership working, including shared decision making between professionals, patients and family carers.
