Entry Requirements
The module is aimed at registered health care professionals currently working in an appropriate area of practice. Normally students may enter Level 6 study if they have completed a qualification at Level 5, equivalent to a Diploma (120 credits at Level 5).
Course Overview
The aims of the module are to demonstrate a systematic understanding of the case management of patients' health care needs across primary and community settings.
This module will enable practitioners working in community and primary care settings to develop the skills and knowledge required to undertake the case management of individuals and families with a range of health needs.
Approaches to successful integration of multi-agency and multi-professional working as part of a case management approach to care will be examined.
