Application form for Health CPD courses
This is the only application form that is acceptable for consideration by the Health CPD team. We will not accept any old versions that were available previously.
Some courses require supplementary forms.
Non-medical prescribing courses
If you are applying to one of the following courses you will need to fill in a Supplementary Admissions Form as well as the Self-Disclosure Form.
- Non medical prescribing for physiotherapists and podiatrists
- Supplementary prescribing for radiographers
- Non medical prescribing for pharmacists
This form enables us to make sure that all Department of Health and professional body criteria are being met.

Healthcare Mentorship Preparation forms
For the Healthcare mentorship preparation (taught) course you also need to fill in the appropriate form:
- Healthcare mentorship preparation - Group selection form semester A
- Healthcare mentorship preparation - Group selection form semester B
- Toolkit Completion Form
Supplementary forms must be submitted with the full CPD Health application form.
