Service user experience of integration in long-term conditions service delivery

Objectives

Despite integrated healthcare delivery being a major policy goal, there is a lack of evidence around the integration of long-term condition services. Due to the complexity of integration, measurement of effectiveness poses significant challenges.

The British Thoracic Society and General Practice Airways Group state that the only way to measure the effect of integrated services is through patient experience.

This proposal will seek to explore the impact of integration of long term conditions services on patient experience and identify patient reported outcomes that indicate the effectiveness of models of integration.

Aims

  • Scope the various models of integration, both vertical and horizontal, within Hertfordshire and Essex
  • Explore the patient experience of the various models of integration.
  • Develop a tool to measure patient experience of integrated long-term conditions services
