Who should attend this course?
The Managing long-term conditions course is suitable for all qualified nurses working in secondary or primary care.
Course content
This course is designed to promote an understanding of the impact of long term conditions on the physical and psychosocial health of individuals and their carers.
You will develop an understanding of the evidence base for approaches to long term conditions disease management.
You will also develop the skills and knowledge to enhance service delivery.
This course gives you the opportunity to explore the impact of long term conditions on the individual, their family and society.
It is divided into four themes:
- The incidence and impact of LTC
- Psychosocial aspects to living with a LTC
- Policy responses to LTC
- Evidence based approaches to the management of LTC
You will be encouraged to challenge professional assumptions regarding the needs of individuals and families affected by long term conditions.
